A popular way to unwind after a long day may come with a hidden cost. A new study published in the journal Alzheimer’s & Dementia found that adults who reported frequent TV watching in the late 1980s had smaller volumes decades later in brain regions known to shrink early in Alzheimer’s disease, along with more damage to the brain’s internal wiring.

Researchers tracked more than 1,700 middle-aged adults over roughly 22 years. The twist: people who sat just as much, but at a desk job instead of in front of a screen, showed the opposite pattern, with larger volumes in several of those same brain regions. Both groups were sedentary. Their brains told different stories.

That gap suggests the type of sedentary activity may matter as much as the total time spent seated. This single study cannot prove TV watching causes brain shrinkage, only that the two are connected.

This study drew on data from the Atherosclerosis Risk in Communities study, a long-running research project that has followed tens of thousands of adults across four U.S. communities since the late 1980s. For this analysis, 1,712 participants who attended both an early visit in 1987 to 1989 and a later visit in 2011 to 2013 were included. None had dementia at the time of brain scanning. About 57% were women, and participants had an average age of 53 at the start and 76 at the imaging visit.

Early in the study, participants were asked how often they watched television during leisure time and how often they sat during work, with options ranging from “never” to “very often.”

Researchers looked at volumes in brain regions known to shrink early in Alzheimer’s disease, as well as in the brain’s outer layers and deeper structures. They also measured white matter hyperintensities, bright spots on brain scans that signal damage to the brain’s internal communication pathways. More of these spots generally means a higher risk of cognitive decline.

People who reported watching TV very often had smaller volumes in several brain regions, including areas linked to Alzheimer’s disease, compared to those who rarely or never watched TV. They also had a higher volume of white matter hyperintensities, the bright spots tied to small-vessel damage in the brain. These associations persisted even after accounting for participants’ physical activity levels, blood pressure, diabetes, smoking, drinking, and body weight.

Sitting during work told a different story. Participants who reported always sitting at work had less internal brain pathway damage and larger volumes in several brain regions, including the frontal region, which is involved in planning, decision-making, and problem-solving.

One possible reason for the difference, the researchers suggest, is that sitting at work often involves sustained mental effort and goal-directed thinking, which may help maintain brain structure over time. That raises the possibility that cognitive engagement plays a role, though the study did not directly measure how mentally demanding each participant’s job or TV habits actually were.

Sex played a notable role in the findings. Among men, frequent TV watching was broadly linked to smaller volumes across multiple brain regions, including areas involved in emotion, language, visual processing, and memory. Men who sat more at work also showed larger volumes in specific frontal brain regions. In women, these associations were largely absent or much weaker.

Researchers cannot pinpoint the exact reasons for this difference from this study alone. One possibility raised in the paper is that women may watch TV in shorter, more interrupted stretches than men. Another is that men and women may differ in the types of activities they pursue during leisure time. Hormonal differences may also play a role, though the study was not designed to test that directly.

One of the more revealing aspects of the study is that factoring in physical activity did not meaningfully change the results. The brain differences tied to TV watching and occupational sitting appear to be at least partially independent of how much a person exercises.

Researchers also ran multiple rounds of additional testing to check whether the findings held up after excluding participants with early memory problems or pre-existing vascular brain conditions, and after accounting for a gene variant tied to Alzheimer’s risk. For most of the key findings, the results remained consistent.

For dementia prevention research, which has long focused on what people do when they’re active, this study redirects attention to what happens during the hours people spend seated, a substantial portion of the day for older adults. Sitting less is good advice. This study suggests what fills that sitting time matters too.
